Poor sleep, different cause.
Imagine two different people saying “I slept poorly last night.”
The first lies awake until 1am. They’re body tired, but their mind is racing. Once asleep, they stay asleep. It takes them until mid-morning to feel functional and they're hyperaware of it.
The second falls asleep almost instantly. They sleep eight hours, yet they still begin their morning exhausted, like they've already been up for hours.
Same langue, same perceived situation—but their cortisol is following opposite rhythms. And all the tips that could help the first person won’t do anything for the second.
Focus on the shape of the rhythm, not the number.
A single cortisol reading says very little on its own. Values can vary widely between healthy people, and one number in isolation can only be read against a general population range, which may not be the best fit when it comes to your unique lifestyle and habits.
Instead, focus on the relationship between your morning and evening cortisol readings. Valuable information can lies in: how much cortisol rises after waking, how far it falls by night, and the difference between the two numbers. That slope often corresponds to the kind of night you're having, and why.
Four distinct rhythms that can shape sleep.
Standard: A sharp morning rise that declines steadily to an evening low. Cortisol release is naturally low during slow-wave sleep,¹ so in a standard rhythm, the bottom of the curve and the deepest part of the night line up. Your system is quiet when it's time to rest.
Note: sleep can still be poor here—there are plenty of things that disrupt sleep that have nothing to do with cortisol. But if your rhythm is standard and you're still sleeping badly, that's genuinely useful information that can guide you to examine other factors.
Elevated: Cortisol is high at night when it should be low. Of the four, this has the most direct sleep evidence behind it: higher pre-sleep cortisol has been shown to forecast shorter, less efficient sleep that same night.²
This is the first person above. They're wired at bedtime with a racing mind, and often experience a second wind that arrives after dinner and doesn’t taper off.
If this is your pattern, it’s worth examining your caffeine timing, how hard you're training, and if work trickles into the evening. All three can strain a rhythm that's supposed to be descending come nighttime.
Flat: Little change between your morning and evening values—meaning no real spike and no true floor. This pattern is often a downstream signal, rather than the root cause. Poor nights are frequently followed by a flatter rhythm the next day,² and the effect accumulates across weeks. A flatter daily rhythm has also been associated with persistent fatigue in a cohort of 4,299 adults.³
This is the second person who sleeps enough but still doesn’t feel restored.
If this is your pattern, look into cumulative inputs, rather than single nights. This can include recent sleep debt, how consistent your wake time is, and overall training load. Flat rhythms rarely respond to one good night of sleep, because they generally weren't built by one bad one.
Inverted: Cortisol is low in the morning and climbs toward its highest point at night. This is the standard rhythm, reversed. Your mornings probably feel foggy and flat until the afternoon, when your energy begins to build. By evening, you feel alert and productive.
This pattern shows up most often alongside disrupted scheduling. A 2025 systematic review of healthcare shift workers found that lower cortisol on waking was associated with poorer sleep quality.⁴
If this is your pattern, consider how timing anchors your rhythm before anything else. Wake time and meal timing especially. Given how heavily scheduling can impact this rhythm, it’s important to assess first before looking into other causes.
Test, then assess.
